■ Suspension takes effect on the first day of the month following submission. Please submit this form by the 25th of the month before the requested suspension start date.
■ An ONU maintenance fee of JPY 396 per month (tax included) will apply during the suspension period.
■ The maximum suspension period is 12 months. If you wish to suspend service for more than 12 months, you must submit another suspension request.
■ CATV service will be unavailable during the suspension period. For Internet service, e-mail and website space will be unavailable. Charges for paid optional services will continue to apply.

■ If your current subscription is the 10G Course on the 2YEARS Plan:
• The dedicated ONU for the 10G Course must be removed. A removal fee of JPY 6,600 (tax included) will apply.
• If you select the 10G Course on the 2YEARS Plan again when restarting service after suspension, an installation fee of JPY 6,600 (tax included) for the dedicated ONU will apply.
